    Registration Instructions

  • Know the name of each student before you begin
  • Click on the link below that applies to you (Group of Individual)
  • Provide all of the requested information and fill in each blank. Submit the form when it has been completed
  • Upon completion, print out the Medical Release From and Application. Copy and distribute the form to each student and have them fill it out.
  • Print, copy and distribute the Uplift Dress Code and review it with your group before arriving. Make sure each student understands it.
  • Mail in the completed forms with the deposit for each student. You may pay the deposit online instead.
  • Notify the Uplift office of any changes.
