Know the name of each student before you begin
Click on the link below that applies to you (Group of Individual)
Provide all of the requested information and fill in each blank. Submit the form when it has been completed
Upon completion, print out the Medical Release From and Application. Copy and distribute the form to each student and have them fill it out.
Print, copy and distribute the Uplift Dress Code and review it with your group before arriving. Make sure each student understands it.
Mail in the completed forms with the deposit for each student. You may pay the deposit online instead.
Notify the Uplift office of any changes.
